YOUR PRACTICE NEEDS YOU
Practice Health Champions volunteer with their GP Practice to deliver health and wellbeing activities to patients and the wider community. Practice Health Champion attended training courses which helped to develop their knowledge, skills, experience and passion. Practice Health Champions volunteer with the practice for approximately 3 hours per week. The Practice Health Champions and PPG members meet regularly with GP practice staff to identify and explore health concerns and issues which affects patients of the practice. They also develop and run our weekly coffee mornings, walking groups, chair aerobics and talk therapy groups,

PATIENT ENGAGEMENT AND PARTICIPATION
We held our first patient and public engagement event on Thursday 13th December 2012 in the former cafe area of Westbourne Green.
the idea was to improve patient knowledge about services that are available to them in their own community

The Farcliffe Family Centre supports local families with under 5s by providing information, advice and services. They help parents returning to work, finding child-care, family support, family health and much more.
Carers Resource give tailored support and information to unpaid carers and vulnerable people. It is an independent, award-winning Yorkshire charity which is open to everyone and offers information, practical and emotional help to enable them to cope. If you look after a family member, friend or neighbour who has a physical disability, mental health problem, drug/alcohol addiction, or who needs extra help due to their age and infirmity, then you are a carer. Carers’ Resource is a specialist service working with carers to help find ways to improve their lives and the lives of those they care for.
The Patient Engagement Team: Rob and Sid supported Picton in organising this event, and in fact held their own stall encouraging patients to sign up to their own practice participation groups, as well as promoting the choose well campaign.
Creative Support is a provider of person centred social care services for people with learning disabilities, mental health and other needs. They provide a range of services to meet the differing needs, including supported living, supported housing, residential care, community and home support, floating support, extra-care and day services. They work with people who have a wide spectrum of needs, ranging from those who require short term support to regain their independence, to people with complex needs who require intensive support to enjoy a good quality of life.
Smoking Cessation advisors attended, to raise awareness about the effects of smoking cigarettes, chewing tobacco related products and smoking Hookah and the ever increasing numbers of cafes and bars which offers different services, such as Shisha sessions.
The advisors sign posted patients to free smoking cessation advice clinics available in Westbourne Green and other venues across the city.
The event was a success and some of the comments from you in the suggestion box state that you would like to know more about what is going on in the community, and more about self-care. With this in mind we are now holding weekly Coffee Mornings, Chair Aerobics sessions, Talk Therapy sessions, and in the summer of 2019 we trained 3 walk leaders, who will be starting walking groups in the early spring.
Our Champions are available to listen to any other ideas you may have about what you would like to see or become more engaged in. Maybe you would like to become a volunteer and work with them?
